Painted white, with a blue and a yellow taped stripe, an applied Bell Atlantic logo on each side, and mounted on a wooden base. A gold tone metal pen holder is also attached to the wooden base and holds a black ball point pen. The pen is not marked but I do believe it to be the original pen. There are no identifying marks on the hat(other than the logo) or base. Brown felt as been affixed to the bottom of the base to protect surfaces. Base measures 4" square x 3/4" h(4-1/4" h to top of hard hat) Very good condition.
